About the Role

We are seeking a highly skilled and compassionate Licensed Pharmacist to join our team at Lawtons. As a Pharmacist at our retail pharmacy, you will be responsible for providing exceptional patient-centered care and exceeding patient expectations.


Key Responsibilities


  • Provide clinical services to patients, including chronic health condition monitoring and management, medication reviews, health risk assessments, prescription renewals or modifications, minor ailments, and other prescribing services as appropriate to practicing top of scope in your province.




  • Initiate care plans, follow up, and complete associated documentation.




  • Collect relevant patient, drug, and medical condition information.




  • Identify and resolve actual and potential drug-related problems.




  • Monitor and optimize medication adherence.




  • Provide patient counselling tailored to the needs and concerns of the patient, ensuring comprehensive understanding of the medication.




  • Understand all pharmacy offerings and provide each patient with a personalized interaction and intervention if appropriate.




Requirements


  • Bachelor of Science in Pharmacy or Doctor of Pharmacy from a recognized institution.




  • Licensed Pharmacist in the province of responsibility.




  • Authorization to Administer Injections.




  • Excellent attention to detail, ability to multi-task, and work independently in a fast-paced environment.




  • Adhere to and implement all applicable company standards.
