Community Health Nurse

Vancouver Coastal Health is seeking a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to join the Community Transition Care Team in Vancouver, BC.
Key Responsibilities- Provide holistic nursing care in an interdisciplinary clinic setting to designated clients with a variety of acute and chronic illnesses/diseases, including mental health and substance use, communicable diseases, and/or general health issues.
- Use the principles of harm reduction, the Clinic Nurse performs physical and mental health assessments, establishes, coordinates, reviews, and updates care plans, observes, monitors, and evaluates client progress and response to treatments, makes referrals and supports transition between services as needed.
- Provide education and counselling, and administers prescribed treatment, medication, and intravenous therapy in a safe and effective manner as required.
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year recent related clinical nursing experience providing care and/or treatment services to the designated client population through a harm reduction, holistic mental health and substance use lens, or an equivalent combination of education and experience.
The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
