We are seeking a skilled Community Service Worker to join our team at BeConnected Support Services LTD.

Job Description:

This role involves working closely with clients to assess their needs and develop personalized plans to support their well-being. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Client Assessment and Case Management: Conduct thorough assessments of clients' needs and create tailored plans to address their challenges.
  • Liaison and Collaboration: Build relationships with other social services agencies and healthcare providers to ensure comprehensive support for clients.
  • Counselling and Workshops: Provide individual and group counselling sessions, as well as lead life skills workshops to empower clients.
  • Reporting and Evaluation: Prepare accurate reports on client progress and contribute to program evaluations to drive continuous improvement.
Requirements:
  • Education: Secondary (high) school graduation certificate
  • Experience: 1 year to less than 2 years of experience in a related field
  • Language: Fluency in English
  • Work Schedule: 35 hours per week
