About the Role

VON Canada is seeking a skilled Continuing Care Assistant to join our team in a home care and visiting care agency setting. As a Continuing Care Assistant, you will provide high-quality care to patients in their homes, focusing on their physical, emotional, and social well-being.

Key Responsibilities
  • Personal Care: Assist patients with bathing, dressing, and grooming, ensuring their dignity and comfort.
  • Mobility and Transfers: Safely transfer patients between beds, wheelchairs, and other equipment, promoting their mobility and independence.
  • Meal Preparation and Feeding: Prepare and serve nutritious meals, ensuring patients' dietary needs are met.
  • Housekeeping and Laundry: Maintain patients' living spaces, including cleaning, laundry, and organization.
  • Communication and Companionship: Engage with patients, listening to their concerns, and providing emotional support.
  • Medication Administration: Assist patients with medication administration, following established protocols.
  • Documentation and Reporting: Accurately document patient care, including progress notes, medication administration records, and other relevant information.
Requirements
  • Education: Secondary (high) school graduation certificate.
  • Experience: Experience an asset.
  • Certifications: Continuing Care Assistant Certification, First Aid Certificate, CPR Certificate.
  • Security and Safety: Criminal record check, Child Abuse Registry check, Immunization records, Vulnerable sector check.
  • Work Conditions and Physical Capabilities: Repetitive tasks, Physically demanding, Attention to detail, Bending, crouching, kneeling.
  • Personal Suitability: Efficient interpersonal skills, Excellent oral communication, Flexibility, Organized.
