We are committed to providing exceptional patient care and we are seeking a qualified Registered Nurse to join our team as a Patient Care Coordinator.
As a Registered Nurse with Fraser Health, you will have the opportunity to work in a dynamic and supportive environment where you can make a real difference in the lives of our patients and their families. Your expertise and compassion will be valued and respected as you provide high-quality patient care and coordinate with other healthcare professionals to ensure seamless transitions and optimal outcomes.
This is a full-time permanent position based at one of our acute care hospitals or community health centers. As a Registered Nurse, you will have the autonomy to prioritize your work and manage your caseload, ensuring that each patient receives the highest level of care and attention. You will work collaboratively with our interdisciplinary teams to develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
You will have the opportunity to work with a diverse range of patients and families, including those with complex medical conditions, mental health concerns, and social determinants of health. Your role will involve assessing patients' physical, emotional, and spiritual needs, and developing and implementing nursing interventions to promote health, prevent illness, and improve quality of life.
In this role, you will also have the opportunity to participate in education and research initiatives, contribute to quality improvement projects, and engage in peer-to-peer mentoring and coaching. We offer a comprehensive compensation package, which includes a competitive salary of approximately $59.52 per hour, four weeks of vacation time, comprehensive health benefits, and a pension plan.
