Salary: $43,184 - $46,133 per year

Job Description:

Job Overview:

We are seeking an experienced Physiotherapist to join our team as a Patient Care Specialist. As a key member of our multidisciplinary team, you will play a vital role in providing comprehensive physiotherapy services to our patients in their homes and community settings.

Key Responsibilities:

  • Assess and determine patient care needs and eligibility for physiotherapy services
  • Provide access and referrals to community services and resources
  • Engage with patients, caregivers, and other healthcare practitioners to ensure seamless care coordination
  • Develop and implement individualized treatment plans to achieve optimal patient outcomes
  • Maintain accurate and up-to-date patient records, adhering to regulatory requirements and organizational policies
  • Foster strong relationships with patients, families, and caregivers to promote trust and confidence in our services
  • Participate in quality improvement initiatives and contribute to the development of best practices in patient care
  • Collaborate with interdisciplinary teams to provide holistic care and support to patients

Requirements:

  • Degree or diploma in Physiotherapy from a recognized institution
  • Certificate of registration with the College of Physiotherapists of Ontario
  • Minimum 2 years of experience in a physiotherapy role, preferably in a community-based setting
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Strong problem-solving and critical thinking skills, with a focus on patient-centered care
  • Ability to maintain confidentiality and handle sensitive information with discretion
  • Fluency in English and French is an asset
