Job Summary
Vancouver Coastal Health is seeking a Physiotherapist 2 to join the ABI Transitional Housing team at George Pearson Centre in Vancouver, BC. This innovative position will support a regional ABI Transitional Housing Program, co-designed with specialists from across VCH, including brain injury, rehabilitation, substance use, mental health, and safety.
Key Responsibilities
- Plan, deliver, evaluate, and provide care coordination to address the needs of clients and families experiencing physical or functional impairment and acute/chronic disease within community, ambulatory, and institutional settings.
- Work without general supervision in a team-based environment in partnership with the client, family members, or significant others.
- Utilize educational resources and other healthcare professionals/providers as needed to deliver direct care through the development, implementation, and adaptation of a care plan.
- Provide care management to assist clients to manage their own healthcare/navigate through various services available within the community setting to address the client's functional/physical/social/cognitive/perceptual/environmental needs to maximize capabilities, community participation, recovery from, or adaptation to, illness/impairment.
- Contribute to the organization and delivery of physiotherapy services by participating in program quality assurance initiatives, meetings, research, and education.
Qualifications
Education & Experience
- Current full registration with the College of Physical Therapists of BC (CPTBC).
- Eligible for membership in the Physiotherapy Association of British Columbia (PABC).
- Valid BC Driver's License. Local travel requires the use of a personal vehicle.
Knowledge & Abilities
- Broad knowledge of physiotherapy theory and practice within a client and family-centered model of care.
- Broad knowledge of other healthcare disciplines and their role in client care.
- Demonstrated ability to function as an effective team member.
- Demonstrated ability to develop rapport, trust, and ethical relationships with clients/families, family care providers, and other healthcare professionals.
- General knowledge of Trauma-Informed Practice and Harm Reduction approaches and Strengths-based care.
- Demonstrated ability to promote Indigenous Cultural Safety.
- Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations.
- Knowledge of acute, chronic disease, palliative, and health management and self-management support.
- Knowledge of community resources available for client/family support and related health services.
- Knowledge of transitions through continuums of care.
- Demonstrated ability to teach and empower clients to assume control over their health.
- Demonstrated ability to communicate effectively with co-workers and clients both one-on-one and in groups.
- Demonstrated ability to work independently and in collaboration with others.
- Demonstrated ability to provide effective consultation.
- Demonstrated ability to establish workload priorities in collaboration with others in a complex healthcare environment.
- Demonstrated ability to recognize when referral to another healthcare professional is required.
- Basic knowledge of research process and methodology.
- Physical ability to perform the duties of the position.
- Skill in the use of equipment and in the techniques appropriate to physiotherapy treatment.
- Basic computer literacy.
