Job Title: Outreach Support Worker

We are seeking a highly motivated and compassionate Outreach Support Worker to join our team at Creating Hope Society of Alberta. As an Outreach Support Worker, you will play a vital role in providing support and services to individuals experiencing homelessness.

Job Summary:

The successful candidate will be responsible for providing outreach services to individuals in need, including assessment, case management, and connection to community resources. You will work collaboratively with our team to develop and implement individualized plans to support clients in achieving their goals.

Key Responsibilities:
  • Provide outreach services to individuals in need, including assessment, case management, and connection to community resources.
  • Develop and implement individualized plans to support clients in achieving their goals.
  • Work collaboratively with our team to provide comprehensive support services.
  • Assess client's relevant skill strengths and development needs.
  • Conduct follow-up assessments to monitor client progress.
  • Develop, co-ordinate and implement the delivery of specific services within the community.
  • Provide suicide and crisis intervention.
  • Resolve conflict situations.
  • Liaise with other social services agencies and health care providers involved with clients.
Requirements:
  • Post-secondary education in a related field (e.g. social work, psychology, counseling).
  • Experience working with vulnerable populations, including individuals experiencing homelessness.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
