Job Summary

VHA Home HealthCare is seeking a skilled Occupational Therapist to join our team. As an Occupational Therapist with VHA, you will have the opportunity to work with diverse client groups across Southern Ontario and the capital region. Our Occupational Therapists work within a continuous quality improvement framework, reflecting VHA's core beliefs and mission of services.

About the Role

This is a challenging and rewarding role that offers the opportunity to work in a dynamic and caring environment. As an Occupational Therapist with VHA, you will be responsible for providing consultation, assessment, and planned interventions for clients. You will work closely with a diverse regional team and have access to beneficial home office programs.

Key Responsibilities

The successful candidate will have the following key responsibilities, but not limited to:
  1. Providing occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the College of Occupational Therapists of Ontario.
  2. Implementing client interventions based on assessment information, client input, and in conjunction with the client's integrated care plan.
  3. Ensuring that services provided are best practice, evidence-based, and evaluated through the routine use of valid and reliable outcome measures.
  4. Ensuring coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
  5. Documenting all assessment findings, treatment plans, progress, and discharge summaries of interventions as required by the profession, referral source, and the organization.
Requirements

The ideal candidate will have a degree in Occupational Therapy from a recognized school of Occupational Therapy. They will also be a member in good standing with the College of Occupational Therapists of Ontario and have a valid driver's license with access to a vehicle.
