Job Summary

The Fraser Health Authority is currently seeking a skilled Case Assistant to support its Acquired Brain Injury Services team in Langley, B.C. This is a Casual opportunity that offers a comprehensive benefits package, generous vacation time, and the chance to make a meaningful difference in the lives of our clients.

Key Responsibilities

  1. Provide case management assistance to a trans-disciplinary team, coordinating services for clients and families, and supporting the team with various administrative duties.
  2. Coordinate contract, financial, and administrative aspects of client services and case management under the direction of managers and clinical staff.
  3. Support clients accessing services or managing urgent issues, ensuring that the case manager is kept informed and reviewing files to obtain historical information.
  4. Research community resources and identify information to assist with client services and case management plans.
  5. Complete initial intake forms, arrange services with community agencies, and escort clients to view potential residential placements.
  6. Advocate on behalf of clients and families, promoting their right to autonomy, dignity, and privacy.
  7. Organize client service meetings, schedule community appointments for clinical staff, and work collaboratively with the team to develop and document care plans.
  8. Monitor client records, maintain spreadsheets, and complete filing and administrative duties.

Requirements

  • Grade 12, Social Services Worker Diploma, and two years of recent experience working with children and adults with physical and/or cognitive disabilities.
  • Valid B.C. Driver's License and access to a personal vehicle for business purposes.
