Program Support Coordinator, Acquired Brain Injury Services

Job Summary
The Fraser Health Authority is currently seeking a skilled Case Assistant to support its Acquired Brain Injury Services team in Langley, B.C. This is a Casual opportunity that offers a comprehensive benefits package, generous vacation time, and the chance to make a meaningful difference in the lives of our clients.
Key Responsibilities
- Provide case management assistance to a trans-disciplinary team, coordinating services for clients and families, and supporting the team with various administrative duties.
- Coordinate contract, financial, and administrative aspects of client services and case management under the direction of managers and clinical staff.
- Support clients accessing services or managing urgent issues, ensuring that the case manager is kept informed and reviewing files to obtain historical information.
- Research community resources and identify information to assist with client services and case management plans.
- Complete initial intake forms, arrange services with community agencies, and escort clients to view potential residential placements.
- Advocate on behalf of clients and families, promoting their right to autonomy, dignity, and privacy.
- Organize client service meetings, schedule community appointments for clinical staff, and work collaboratively with the team to develop and document care plans.
- Monitor client records, maintain spreadsheets, and complete filing and administrative duties.
Requirements
- Grade 12, Social Services Worker Diploma, and two years of recent experience working with children and adults with physical and/or cognitive disabilities.
- Valid B.C. Driver's License and access to a personal vehicle for business purposes.
