About the Role

We are seeking a highly skilled and experienced Patient Care Manager to join our team. As a Patient Care Manager, you will be responsible for leading a team of Personal Support Workers to provide high-quality patient care in a home setting.

Key Responsibilities:
  • Lead a team of Personal Support Workers to deliver exceptional patient care in a home setting
  • Travel to patients' homes in community and facility settings to provide support and guidance
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Assess patient needs and develop personalized care plans to achieve optimal outcomes
  • Manage staff performance, conduct regular appraisals, and implement progressive disciplinary actions as needed
  • Gather feedback from patients and employees, analyzing key indicators such as caseload volume, staff turnover, occurrence rates, satisfaction levels, risk levels, and performance appraisal completion rates
  • Address patient concerns and complaints in a timely and professional manner
  • Promote Spectrum Health Care through participation in local healthcare committees and community events
  • Participate in ongoing continuing education to enhance personal development and stay up-to-date on industry best practices
Requirements
  • Current registration with the Colleges of Nurses of Ontario in good standing
  • Minimum three years' experience in leadership roles, preferably in a community setting
  • Exceptional interpersonal skills and ability to handle difficult situations effectively
  • Excellent written and verbal communication skills, with proficiency in Microsoft Word, Excel, and proprietary software
  • Valid driver's license and access to a reliable vehicle
