Job Title: Clinical Lead - Care Team

VHA Home HealthCare is a leading provider of home care services in Ontario, with a team of over 3,000 dedicated professionals. As a Clinical Lead - Care Team, you will play a key role in providing high-quality, client-centered care to those who need it most in their homes, schools, retirement homes, and other community settings.

Key Responsibilities:
  • Provide clinical support, education, and training to Personal Support Workers (PSWs) within dedicated geographical teams.
  • Develop and implement client care plans based on individual needs and authorized services.
  • Conduct home visits and monitor client safety.
  • Teach PSWs evidence-based practice, including special functions and delegation.
  • Participate in clinical-focused committees and working groups.
Requirements:
  • Training and qualifications as a Registered Practical Nurse (RPN) or Registered Nurse (RN) with College of Nurses of Ontario (CNO) registration.
  • Member in good standing of the College of Nurses; membership in the appropriate professional association an asset.
  • Minimum of 2-3 years of experience in community health preferred.
  • Demonstrated ability to assess client needs and formulate care plans.
  • Superior interpersonal, verbal, and written communication skills.
