Job Overview

Sunrise Senior Living is seeking a dedicated and compassionate Wellness Nurse to join our team. As a key member of our care team, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities
  • Conduct regular wellness visits for all residents to identify their ongoing needs and services.
  • Collaborate with the Resident Care Director to evaluate and document resident care plans and changes in status.
  • Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
  • Communicate effectively with residents' families and responsible parties to ensure their medical needs are being met.
  • Assist in maintaining accurate and up-to-date resident wellness files, adhering to Sunrise policies and federal and state regulations.
  • Provide clinical support and assistance to community team members as needed.
Qualifications
  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Excellent communication and interpersonal skills, with the ability to teach and guide team members.
  • Competent in organizational, time management, and problem-solving skills.
