Job Title: Forensic Community and Liaison Social Worker

We are seeking a highly skilled and experienced Forensic Community and Liaison Social Worker to join our team at PHSA. As a key member of our team, you will be responsible for providing high-quality social work services to individuals with complex mental health and substance use challenges.

Key Responsibilities:
  • Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop integrated treatment plans.
  • Provide community case coordination and advocacy services to support clients in accessing community resources and services.
  • Maintain accurate and concise documentation of client information and provide written reports as required.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
  • Facilitate access to psychiatric and social services through referrals and site visits.
Requirements:
  • Bachelor's degree in Social Work from an accredited school.
  • Two years of recent experience working in a mental health setting.
  • Full registration with the British Columbia College of Social Workers (BCCSW).
  • Valid BC Driver's License.
  • Current Cardiopulmonary Resuscitation (CPR) Certificate.
