About the Hospital at Home Program
Vancouver Coastal Health is seeking a Registered Nurse to join the Hospital at Home Program team at Vancouver General Hospital. This innovative program provides hospital-level care to patients in the comfort of their own homes.
Key Responsibilities
- Collaborate within an interprofessional team to provide nursing care to patients requiring acute level care in their homes.
- Assess, identify, plan, implement, and evaluate the nursing care required to assist patients with acute health care needs in the home and/or hospital setting.
- Develop, implement, evaluate, and revise care plans, participate in discharge planning, function as a patient advocate, and teach patients and their caregivers about illness/injury and overall health promotion and wellness.
Qualifications
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' experience, including two (2) years' recent, related acute medical/surgical experience or an equivalent combination of education, training, and experience.
