Job Summary

This part-time permanent position involves working 8-hour shifts, including evenings and weekends, with opportunities for professional growth and development. As a key member of our interdisciplinary team, you will be responsible for assessing, planning, and delivering skilled nursing care to residents, while also collaborating with colleagues to ensure seamless care transitions.

Key Responsibilities:
  • Patient Assessment and Planning: Conduct comprehensive assessments of residents to identify their unique needs and develop individualized care plans that prioritize their comfort, safety, and well-being.
  • Care Delivery: Provide direct patient care, administering medications, performing procedures, and collaborating with healthcare teams to ensure seamless care transitions.
  • Leadership and Guidance: Lead by example, demonstrating expertise and confidence in your practice, while guiding and supporting less experienced nurses in the delivery of high-quality care.
  • Communication and Collaboration: Foster strong relationships with patients, families, and healthcare teams, communicating effectively through written and verbal interactions.
  • Education and Professional Development: Participate in ongoing education and training to enhance your knowledge and skills, staying up-to-date on best practices and research-based interventions.
Qualifications and Requirements:
  • Education: Diploma in Nursing or Baccalaureate Degree in Nursing.
  • Licensure: Current registration with the College of Nurses of Ontario.
  • Experience: Previous experience in rehabilitation or long-term nursing, preferably in gerontological care.
  • Skill Set: Strong interpersonal communication skills, proficiency in computer applications (WORD, Excel, email, internet use), and experience with electronic documentation.
  • Certifications: Basic Cardiac Life Support (BCLS) certification and current AIS Certification.
