About the Role


We are seeking an experienced Occupational Therapist to join our team as a Home Care Specialist. In this role, you will work with clients in their homes, providing personalized therapy services to improve their well-being and quality of life.


Key Responsibilities



  • Conduct comprehensive assessments of clients' needs and develop tailored therapy plans
  • Deliver occupational therapy services in clients' homes, including development of health promotion programs, equipment prescription, and supply management
  • Collaborate with inter-disciplinary healthcare teams to achieve holistic treatment goals and service plans
  • Build strong relationships with clients, families, and caregivers to ensure seamless care coordination

Requirements



  • Degree in Occupational Therapy from a recognized post-secondary program
  • Valid driver's license and reliable vehicle
  • Experience working in home care or community healthcare settings (an asset)
