About the Opportunity
We are seeking a compassionate and dedicated Transitional Care Assistant to join our team at Shannex Incorporated. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care and support to our patients.
Key Responsibilities:
- Assist patients with daily living activities, such as bathing, dressing, and mobilizing, according to their care plan.
- Collaborate with patients and other team members to develop and implement individualized care plans.
- Assist with light housekeeping duties and maintain a clean and safe environment.
- Perform delegated nursing acts according to company policy and scope of practice.
- Communicate and report relevant information regarding patient care or safety to Supervisor.
About You
We are looking for a qualified and experienced Continuing Care Assistant who is registered in Nova Scotia's CCA registry. You must have a valid Continuing Care Certification or Provincial Personal Care Worker course or equivalent. Previous experience working with elderly in long-term care or a senior living environment is an asset.
