Job Title: Licensed Practical Nurse

Job Summary:

We are seeking an experienced Licensed Practical Nurse to provide high-quality patient care in our complex continuing care unit. As a key member of our healthcare team, you will be responsible for delivering compassionate and individualized care to our patients, collaborating with physicians and other healthcare professionals to ensure seamless care coordination.

Responsibilities:

  • Provide patient-centered care to patients with complex needs, including those with chronic illnesses and geriatric conditions.
  • Develop and implement individualized care plans, taking into account each patient's unique needs and goals.
  • Collaborate with the interdisciplinary care team to ensure comprehensive care coordination and effective care delivery.
  • Administer medications and treatments as prescribed by physicians, ensuring accurate and timely administration.
  • Monitor patient vital signs and report any changes or concerns to the care team.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Certified Licensed Practical Nurse (LPN) with current registration with the College of Licensed Practical Nurses of Manitoba.
  • Experience working in complex continuing care, geriatrics, or chronic illness care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to prioritize tasks and manage time effectively, with a focus on delivering high-quality patient care.
  • Strong problem-solving and critical thinking skills, with the ability to adapt to changing situations.
  • Willingness to learn and participate in ongoing education and training to enhance patient care and outcomes.
