Job Summary
Reconnect Health Services is seeking a skilled Mental Health Support Professional to join our team. This is a part-time, evening shift position that requires a unique blend of clinical expertise and community engagement.

About the Role
As a Mental Health Support Professional, you will provide on-site assessment, intake, and discharge planning for clients in West Toronto. You will work closely with a clinical program manager and collaborate with other mental health service providers, mobile crisis teams, hospitals, and community workers to ensure comprehensive support for our clients. Your responsibilities will include:
• Providing individualized on-site supports, including addressing immediate short-term housing needs, accessing financial aid, and assessing medical supports;
• Liaising and collaborating with other mental health and justice service providers, mobile crisis teams, hospitals, physicians, community workers, substance abuse programs, landlords, and family members on behalf of clients;
• Coordinating client access to short-term crisis beds;
• Participating in household tasks, including cleaning and domestic routines;
• Attending bi-weekly team meetings;

Requirements
To be successful in this role, you will require:
• A university degree or community college diploma in social services or a related field;
• Minimum two years of experience in direct service delivery to adults with serious mental illness and addictions;
• Knowledge of mental illness and addictions, the mental health system, and other concepts related to the effects of serious mental illness and addictions;
• Experience in provision of residential/housing supports to adults with serious mental illness and addictions;
• Knowledge of housing, healthcare, and community resources;
• Applied computer skills, including Microsoft Office, Client Information Systems, and web browsing;
• Fluency in a second language is an asset;
