Job Title: Unit Coordinator

At Lakeridge Health, we are seeking a highly skilled and experienced Unit Coordinator to join our team in the Emergency Department. As a key member of our inter-disciplinary team, you will play a vital role in ensuring the smooth operation of our department.

Key Responsibilities:
  • Engage in daily discharge rounds for admitted patients, maintaining thorough output and identifying opportunities for improvement.
  • Facilitate appropriate discharge planning and coordinate the transfer of patients to other sites as required.
  • Work closely with the inter-disciplinary team, including nurses, therapists, and social workers, to ensure seamless patient care.
  • Develop and maintain data accountability, ensuring accurate and timely reporting.
  • Act as a steward for developing and facilitating family-centered care approaches.
Requirements:
  • Registration with the College of Nurses of Ontario (in good standing).
  • Current BCLS and ACLS with Medical Directives.
  • Recent and relevant Charge Nurse experience (within the last 2-3 years).
  • Significant and recent experience in an Acute Medical Unit (within the last year).
  • Recent experience in Emergency/Critical Care/Intensive Care nursing (preferred).
  • Proven experience with staff scheduling, patient flow, and patient assignments.
  • B.Sc.N (preferred).
  • Demonstrated leadership, teaching, and mentorship skills.
  • Effective organizational, planning, prioritization, and time management skills.
  • Demonstrated ability to deal with multiple competing demands and maintain composure in stressful situations.
  • Proven ability to work well independently and demonstrate well-developed critical thinking skills.
  • Ability to collaborate with Lakeridge Health Patient Flow, inpatient clinical program interdisciplinary teams, and community partners for quality-based patient outcomes.
