About the Role

We are seeking a highly skilled Licensed Practical Nurse to join our community nursing team. As a Community Nursing Specialist, you will play a vital role in delivering high-quality care to patients in their homes.

Job Description

  • Provide scheduled visits to patients in the community to assess and meet their individual needs.
  • Develop and implement personalized care plans that cater to each patient's unique requirements.
  • Collaborate with patients and their families to ensure seamless care delivery.
  • Maintain accurate and up-to-date patient records to track progress and identify areas for improvement.

Requirements

  • Registration with the NSCN as a Licensed Practical Nurse in good standing.
  • Valid driver's license and access to a reliable vehicle.
  • Experience in delivering community care is an asset.
  • Reliability, flexibility, and a passion for making a difference in your community.
