Bayshore Home Care Solutions is seeking a Registered Nurse (RN) to join our team in the Greater Hamilton Area. As a Palliative Care Nurse, you will provide skilled clinical care to clients in their homes, utilizing your critical thinking skills to address complex care needs.
Key Responsibilities
- Provide high-quality nursing care to clients in their homes
- Develop and implement individualized care plans to meet clients' needs
- Collaborate with healthcare teams to ensure seamless care transitions
- Conduct thorough assessments and evaluations to identify care needs
- Delegate tasks and functions to support staff and client family members
Requirements
- Current registration with the College of Nurses of Ontario
- Valid driver's license and reliable vehicle
- Updated immunization records and clear criminal background check
- Current CPR certification
- Leap Training and Palliative Certificate considered an asset
