Salary: $750 Signing Bonus + Competitive Hourly Rate
The estimated annual salary for this role is approximately $60,000 - $80,000 based on a 40-hour workweek, depending on experience and location.
Job Summary
We are seeking a Registered Practical Nurse (RPN) to join our team as a Community Care Navigator. In this role, you will be responsible for providing scheduled visits to patients in their homes, working closely with RNs to provide complex care when needed. You will have the opportunity to develop long-term therapeutic relationships with patients and play a key role in helping them remain comfortable in their own homes for as long as possible.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Provide ethical and safe nursing care, using appropriate assessment tools and techniques to assess patients' individual needs
- Execute and continuously monitor care plans, adjusting as required to ensure optimal patient outcomes
- Maintain and update professional patient records related to patient encounters and plans
Required Skills and Qualifications:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Experience in delivering community care is an asset
- Reliability and flexibility, with willingness to travel to and from patient residences within a designated area
- Experience working with vulnerable populations and passion for making a difference in your community
