Job Title: Transitional Case Manager

Canadian Mental Health Association Toronto Branch is seeking a skilled Transitional Case Manager to join our team. As a key member of our Transitional Rehabilitation Housing Program 1 (TRHP 1), you will play a vital role in supporting clients with complex mental health needs.

Job Summary:

The Transitional Case Manager will work collaboratively with our interdisciplinary team to provide comprehensive support services to clients. This includes developing and implementing individualized service care plans, linking clients to community resources, and providing on-site and community-based interventions.

Key Responsibilities:
  • Develop and implement individualized service care plans for clients
  • Link clients to community resources and services
  • Provide on-site and community-based interventions, including support with daily living skills, structured day time activities, and relapse prevention
  • Conduct risk assessments and develop risk management plans
  • Collaborate with the interdisciplinary team to ensure comprehensive client support
Requirements:
  • Undergraduate degree or diploma in social services or a related field
  • Minimum 3 years' experience in direct service delivery to adults with complex mental health needs
  • Strong knowledge of forensic mental health, the criminal justice system, and psychosocial rehabilitation
  • Excellent assessment, service planning, and crisis prevention/intervention skills
  • Ability to work with diverse communities and demonstrate an anti-oppression approach to practice
