We are seeking a compassionate and experienced Healthcare Professional to join our team as a Care Coordinator in Lethbridge. As a key member of our healthcare team, you will be responsible for delivering high-quality care to our patients, collaborating with a multidisciplinary team to create personalized care plans, and monitoring patient progress.
Key Responsibilities:- Assess patients' medical conditions and develop comprehensive care plans.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless patient care.
- Educate patients and their families on health management and self-care strategies.
- Maintain accurate and detailed medical records, documenting patient progress and treatment outcomes.
- Valid Registered Nurse (RN) license or equivalent qualification.
- Previous experience in a clinical or hospital setting, preferably in a leadership or coordination role.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Ability to prioritize tasks, manage multiple responsibilities, and maintain confidentiality.
- A competitive salary range of $85,000 - $110,000 per year, depending on experience.
