Job Title: Case Coordinator

WRHA Community Health Services is seeking a skilled Case Coordinator to join our team.

Job Summary

The Case Coordinator will play a critical role in determining program eligibility and assessing the need for care at home or in a personal care home for the elderly or infirm adult. This position requires strong communication and organizational skills, as well as the ability to work effectively with clients, families, and healthcare professionals.

Key Responsibilities
  • Receive referrals and conduct assessments to determine client care needs.
  • Develop and implement care plans, including statements of client need, objectives, service provision, and evaluation criteria.
  • Coordinate the delivery of professional and non-professional services, including medical rehabilitation and consultative services as necessary.
  • Maintain current case counts and ensure proper submission of statistics.
  • Plan and organize work schedules and manage caseload demands effectively.
  • Participate in educational development and training programs as required.
Requirements
  • Two years of directly related community experience preferred.
  • Previous experience as a Case Coordinator in Community Health Services preferred.
  • Valid driver's license and a vehicle required.
  • Licensure/registration as per professional designation required.
Qualifications and Skills
  • Knowledge and experience with computers required.
  • Effective oral and written communication skills.
