Job Summary:
We are seeking a skilled and compassionate Social Worker to join our team at Lakeridge Health. As a member of our inter-professional team, you will work collaboratively with patients, families, and healthcare providers to provide social work interventions and services.
About the Role:
This position is responsible for providing psychosocial assessments, discharge planning, crisis management, individual and group counseling, education, and facilitating case conferences. You will work closely with patients and their families to support them in adjusting to their health care needs and navigating complex transitions.
Key Responsibilities:
- Provide social work interventions and services to patients and their families
- Conduct psychosocial assessments and develop comprehensive treatment plans
- Work collaboratively with inter-professional teams to coordinate care and facilitate smooth patient transitions
- Develop and implement educational programs and materials to support patient learning and empowerment
- Maintain accurate records and documentation of patient interactions and progress
Requirements:
- Masters of Social Work degree from an accredited program
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers
- Experience working with inpatient mental health populations and provision of care through various modalities
- Demonstrated skills in mental health assessment, crisis management, and knowledge of applicable legislation
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment with a varied caseload
Compensation and Benefits:
This full-time position offers a competitive salary range of $44.67 - $57.90 per hour, commensurate with experience and qualifications. We also offer a comprehensive benefits package, including health, dental, and vision coverage, as well as opportunities for professional development and growth.
Location and Working Conditions:
This position is based at Lakeridge Health Oshawa, one of our major hospital sites. You will be required to work flexible hours, including evenings and weekends, to meet the needs of our patients and families. A valid driver's license and access to a vehicle may be required for some positions.
