About the Role

The Continuing Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves working with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. The Coordinator will assess clients' needs, identify goals, and develop plans to meet those goals, while also authorizing services and making referrals to other resources as needed.

Key Responsibilities
  • Assess clients' needs and develop care plans
  • Authorize services and make referrals to other resources
  • Work with interprofessional teams to facilitate hospital discharge
  • Support community-dwelling clients and their families
  • Maintain ongoing communication with clients, families, and healthcare teams
Requirements
  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and experience in case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Competent with computer use and keyboarding
  • Knowledge of community agencies and resources
