Job Description

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Care Coordinator for Complex Needs in Mental Health. This role involves providing comprehensive assessments and treatment management for individuals with mental health and addictions-related issues.

Responsibilities:
  • Deliver client care according to the College of Licensed Practical Nurses of Saskatchewan
  • Provide ongoing client care utilizing the nursing process of assessment, planning implementation and evaluation
  • Formulate and implement client care plans
  • Safely implement physician's orders
  • Administer medications to clients
  • Maintain current progress records on clients according to legal and ethical standards
  • Orientate, facilitate and reinforce client's involvement in addiction treatment
  • Decide and initiate on course of action in case of medical emergencies
  • Providing supportive counselling to clients, families and/or significant others on typical recovery issues
  • Maintain a therapeutic environment on the unit conducive to the client learning productive patterns of living
  • Providing ongoing health teaching to clients in relation to common health problems and the clients maintaining a healthy lifestyle
  • Co-ordinate individual client appointments and referrals
  • Actively participate with team members in the assessment, planning and evaluation of the client and his care
  • Assisting the public with crisis intervention and information on addiction
  • Perform other duties as outlined on shift duty lists and as assigned by the Nursing Supervisor
Qualifications:
  • Graduate of an accredited school of Licensed Practical Nurses with pharmacological training
  • Licensed Practical Nurse registered in the Province of Saskatchewan
  • Current basic CPR
  • Recent clinical experience, including medication administration and leadership duties
  • Demonstrated ability to attend work on a regular and consistent basis
  • Leadership/Charge experience preferred
  • Strong communication, interpersonal, and organizational skills
  • Good problem solving and decision-making skills
  • Demonstrated use of proper body mechanics and ability to perform heavy lifting
  • Knowledge of/experience with a team-based person-centered care approach
  • Demonstrated ability to work effectively as a member of the interdisciplinary team
