Care Coordinator for Complex Needs in Mental Health

Job DescriptionWe are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Care Coordinator for Complex Needs in Mental Health. This role involves providing comprehensive assessments and treatment management for individuals with mental health and addictions-related issues.
Responsibilities:- Deliver client care according to the College of Licensed Practical Nurses of Saskatchewan
- Provide ongoing client care utilizing the nursing process of assessment, planning implementation and evaluation
- Formulate and implement client care plans
- Safely implement physician's orders
- Administer medications to clients
- Maintain current progress records on clients according to legal and ethical standards
- Orientate, facilitate and reinforce client's involvement in addiction treatment
- Decide and initiate on course of action in case of medical emergencies
- Providing supportive counselling to clients, families and/or significant others on typical recovery issues
- Maintain a therapeutic environment on the unit conducive to the client learning productive patterns of living
- Providing ongoing health teaching to clients in relation to common health problems and the clients maintaining a healthy lifestyle
- Co-ordinate individual client appointments and referrals
- Actively participate with team members in the assessment, planning and evaluation of the client and his care
- Assisting the public with crisis intervention and information on addiction
- Perform other duties as outlined on shift duty lists and as assigned by the Nursing Supervisor
- Graduate of an accredited school of Licensed Practical Nurses with pharmacological training
- Licensed Practical Nurse registered in the Province of Saskatchewan
- Current basic CPR
- Recent clinical experience, including medication administration and leadership duties
- Demonstrated ability to attend work on a regular and consistent basis
- Leadership/Charge experience preferred
- Strong communication, interpersonal, and organizational skills
- Good problem solving and decision-making skills
- Demonstrated use of proper body mechanics and ability to perform heavy lifting
- Knowledge of/experience with a team-based person-centered care approach
- Demonstrated ability to work effectively as a member of the interdisciplinary team
