About the Role
We are seeking a skilled Patient Care Coordinator to join our team at North York General Hospital. In this role, you will provide leadership and support to the inter-professional team in the Reactivation Care Centre Unit 2&3.
Job Summary
This is a Full-Time Permanent position with 8 hours shifts, day shifts, including weekends. The successful candidate will have excellent communication and interpersonal skills, with the ability to work independently and collaboratively as part of a dynamic team.
About the Job Description
The Patient Care Coordinator will be responsible for providing day-to-day assistance and leadership for the inter-professional team. This includes collaborating with patients, caregivers, and the interprofessional team to support discharge planning, participating in unit-based and corporate committees, conducting research and creating presentations relevant to the unit and program, developing systems and processes for effective unit functioning, assisting in the implementation and evaluation of targets, goals, and objectives, supporting staff in utilizing critical thinking skills, and engaging in evidence-based practice.
Requirements
To be successful in this role, you will possess a valid Certificate of Competence from the College of Nurses of Ontario, a Baccalaureate in Nursing required, Masters degree completed or in progress, 3-5 years recent experience in medical/surgical and/or nursing experience with older adult, dementia & cognitively impaired patient population preferred, and 3-5 years recent experience of restorative care and rehabilitative care for the elderly and palliative care services preferred. You will also have 3-5 years leadership experience (Charge Nurse) with good understanding of discharge planning and community supports.

Compensation
The estimated salary for this position is $120,000 per year, based on industry standards for similar roles in the Toronto area.
