Job SummaryWe are seeking an experienced Psychiatric Nurse to join our team at the Raven Song Community Health Centre. As a member of our multidisciplinary team, you will provide direct nursing care to adults with severe and persistent mental illness, as well as concurrent disorders.
Key Responsibilities:- Assess clients' physical and psychosocial needs, develop and implement individualized care plans, and evaluate client responses to care.
- Collaborate with other healthcare professionals to ensure comprehensive care, including family physicians and community agencies.
- Provide education and support to clients and their families on activities of daily living, medication management, and healthy lifestyle choices.
- Maintain accurate and up-to-date client records, participate in quality improvement initiatives, and engage in ongoing professional development.
- Current registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Diploma in Nursing or Psychiatric Nursing.
- At least two years of experience working with individuals with severe and persistent psychiatric illness or an equivalent combination of education, training, and experience.
- Valid BC Driver's license and local area travel may require the use of a vehicle.
