We are seeking a highly skilled and dedicated Pharmaceutical Specialist to join our clinical team at Humber River Regional Hospital. This exciting opportunity involves providing safe and effective medication preparation and distribution to all clinical units and outpatient areas.

About the Role

As a Pharmaceutical Specialist, you will be responsible for the provision of clinical pharmacy services, including review of physician's orders, creation and maintenance of computerized patient medication profiles, IV admixture, and drug information. You will also be responsible for computer provider order entry, clinical pharmacotherapy monitoring, and selective medications dispensing.

Responsibilities
  • Provide high-quality clinical pharmacy services to improve patients' quality of life
  • Review physician's orders and create accurate computerized patient medication profiles
  • Promote safe medication practices through IV admixture, TPN, Pediatric and chemotherapy preparation, drug information, Drug Therapy Monitoring and adverse drug reaction reporting
  • Verify prescriptions in the MEDITECH system, monitor clinical pharmacotherapy, check chemotherapy, and selectively dispense medications
Requirements
  • Bachelor of Science in Pharmacy (Pharm D an asset)
  • Current registration with the Ontario College of Pharmacists (Part A)
  • Previous hospital experience in unit dose and/or IV additive (hospital residency program or equivalent an asset)
  • Ability to work independently and as part of a dynamic team
  • Excellent written and verbal communication skills
  • Strong problem-solving and analytical skills
  • Ability to identify, prevent, and resolve drug-related problems
