Mental Health Program Coordinator

Halton Healthcare is seeking a skilled clinician to join our Adult Mental Health Day Program as a Clinical Case Manager. The successful candidate will possess solid knowledge and experience in working with individuals with mental illness, with a focus on crisis management, symptom stabilization, education, group and individual counseling, and coordination of resources for follow-up after discharge.
Key Responsibilities- Provide assessments and treatment planning for clients, developing and implementing groups and demonstrating strong group facilitation skills.
- Work collaboratively within an interdisciplinary team to provide comprehensive care.
- Liaise with community partners, family physicians, and case managers to advocate for this population.
- Develop programming based on best practice and adapt to the demands of a constantly changing workload.
- Ensure a satisfactory employment and attendance record.
- Current college registration (RN, MSW, OT) is required.
- Strong interpersonal and leadership skills are necessary.
- Knowledge and experience in patient-focused goal setting and the use of outcome measures are essential.
- Program development and group facilitation skills are required.
- Flexibility and the ability to work collaboratively within a team are essential.
- Working knowledge of the Mental Health Act and other related legislation is necessary.
Monday-Friday, hour shifts, 8:30 - 16:30 (5 days). Hours are subject to change based on operational needs.
Compensation$ to $ per hour.
