Physiotherapist 2 - ABI Transitional Housing Job Summary

Vancouver Coastal Health is seeking a Physiotherapist 2 to join the ABI Transitional Housing team at George Pearson Centre in Vancouver, BC. The successful candidate will support a regional ABI Transitional Housing Program that has been co-designed in collaboration with specialists from across VCH, including brain injury, rehabilitation, substance use, mental health and safety.

This is a unique opportunity to work in a community program that emphasizes trauma-informed, harm reduction, and culturally safe care. The Physiotherapist 2 will care for clients during transitions and support outreach, working in partnership with the client, family members, or significant others.

Key Responsibilities:

  • Plan, deliver, evaluate, and provide care coordination to address the needs of clients and families experiencing physical or functional impairment and acute/chronic disease within community, ambulatory, and institutional settings.
  • Work without general supervision in a team-based environment, utilizing educational resources and other healthcare professionals/providers as needed to deliver direct care through the development, implementation, and adaptation of a care plan.
  • Provide care management to assist clients to manage their own healthcare/navigate through various services available within the community setting to address the client's functional/physical/social/cognitive/perceptual/environmental needs to maximize capabilities, community participation, recovery from, or adaptation to, illness/impairment.
  • Contribute to the organization and delivery of physiotherapy services by participating in program quality assurance initiatives, meetings, research, and education.

Qualifications:

  • Current full registration with the College of Physical Therapists of BC (CPTBC).
  • Eligible for membership in the Physiotherapy Association of British Columbia (PABC).
  • Valid BC Driver's License. Local travel requires the use of a personal vehicle.
