Job Summary

Prairie Mountain Health is seeking a skilled Health Information Clerk III to join our team. As a key member of our health information services department, you will play a critical role in ensuring the accurate and timely registration of patients and associated processing on our admission/discharge/transfer (ADT) system.

About the Role
  • You will be responsible for registering patients and processing associated information on the ADT system, adhering to provincial and regional registration guidelines and practices.
  • You will collect complete and accurate demographic and financial data, including provincial health coverage and/or related third-party insurance.
  • You will retrieve clinical health information as required and release information in accordance with relevant legislation and regional policy.
Key Responsibilities
  1. Registration and associated processing on the ADT system, including registrations, admissions, transfers, and discharges.
  2. Collect complete and accurate demographic and financial data, including provincial health coverage and/or related third-party insurance.
  3. Complete all necessary registration forms, as required.
  4. R retrieve clinical health information as required.
  5. Release information in accordance with the Personal Health Information Act and the Mental Health Act and regional policy.
  6. Accurately complete and process Birth Registrations in accordance with the Vital Statistics Act.
  7. FOLLOW downtime procedures for registration of patients / maintenance of ADT Downtime system.
  8. Reconciliation of ADT reports (e.g. midnight census) for admissions/discharges/transfers as well as copying and distribution following outlined procedures.
  9. Coordinate completion and processing of Death Registrations in accordance with the Vital Statistics Act.
  10. Maintenance of Morgue documentation, as required.
  11. Inform funeral homes of release and completion of certificate of death, as required.
  12. Coordinate funeral home and transport agency access to Morgue, as required.
  13. R retrieve, document & lock up patient valuables, as required.
  14. Coordinate and process appropriate bed placement within ADT with facility bed management personnel (i.e. Utilization Coordinators, Care Team Managers/Supervisors or facility designates), as required.
  15. Direct clients to appropriate clinical or treatment areas.
  16. Locate and retrieve records required for provision of care.
  17. R retrieve and return records as required.
  18. Review health records for accuracy and completion, in a timely manner, in accordance with minimum documentation requirements.
  19. Confirm and ensure regional chart sequence.
  20. Adhere to regional record processing practices to prepare and scan patient/clinical reports.
  21. Adhere to regional record management practices and policy for record security, storage and control and for retention and destruction of personal health information.
  22. Investigate and reconcile double health record numbers and overlays.
  23. Reconcile system information and prepare reports on a monthly basis or as required.
  24. Operate switchboard to relay incoming calls.
  25. Page physicians and staff using paging equipment as well as overhead paging.
  26. Assist staff and the general public in a kind and helpful manner.
  27. Attend to various alarms at the Switchboard and notify the responsible department, (e.g. Blood Bank, Pharmacy, all CODES).
  28. Quick, appropriate and immediate response to the Emergency phone, if applicable, following established protocols.
  29. Respond to the buzzers for the various doors throughout the facility, if applicable.
  30. Maintain control of the keys for the facility, as required.
  31. Respond to patient inquiries via telephone.
  32. Comply with Provincial Productivity Standards re: job performance.
  33. Other duties as assigned.
