Position Overview

 

We are seeking an experienced Community Health Nurse to join our team at WRHA Community Health Services.

 

Job Summary:

The successful candidate will provide advanced nursing care to clients in the community, focusing on complex and unpredictable health needs. This role involves independent nursing services, administration of prescribed treatments, leadership within the healthcare team, and coordination of client care and discharge.

 

Key Responsibilities:

  • Provide high-quality nursing care to clients in various settings, including home, school, and community clinics.
  • Develop and implement individualized care plans, taking into account the client's physical, emotional, and social needs.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal outcomes.
  • Maintain accurate and up-to-date records of client interactions and care provided.
  • Participate in education and training programs to enhance professional development and stay current with best practices.

 

Requirements:

  • 2 years of related experience as a Registered Nurse in a community setting.
  • Graduate of an approved Registered Nursing education program.
  • CNA Community Health Nurses Certification or relevant applicable certification preferred.
  • Additional wound care and chronic disease management education preferred.

 

Preferred Qualifications:

  • Knowledge of College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses.
  • Demonstrated effective oral and written communication skills.
  • Ability to prioritize care for individuals or groups and delegate tasks accordingly.
  • Experience working with diverse populations and communities.

 

Work Environment:

This role requires the ability to work independently and as part of a team, with frequent travel to various locations throughout the community. A reliable motor vehicle is required, as well as good physical and mental health.

 

Salary Range:

$65,000 - $85,000 per annum, depending on experience.
