Care Professional

VHA Home HealthCare is seeking a Registered Nurse to provide home health nursing services to diverse groups of individuals, families, and groups. The successful candidate will be responsible for assessing, monitoring, and making care planning/care coordination decisions in accordance with the College of Nurses of Ontario Practice Standards, VHA policies and procedures, and the Standards of Practice in Community Health Nursing.
Key Responsibilities:- Assess and monitor clients to identify their health care needs and develop care plans accordingly.
- Evaluate care plan interventions through ongoing reassessment and evaluation results.
- Ensure the quality of care for clients by identifying and acting on factors that affect practice autonomy and delivery of quality care.
- Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.
- Support client safety through the development of knowledge and skills in accordance with principles of continuous quality improvement.
- Maintain accurate and timely documentation of client interactions and care plans in accordance with the College of Nurses of Ontario standards.
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- Membership of the Registered Nurses Association of Ontario (RNAO) or WeRPN preferred.
- RN diploma/degree from a recognized university or college.
- Valid First Aid or CPR certification.
- Modest level of wound care or venipuncture knowledge and experience an asset.
- Acute care and medical-surgical nursing experience an asset.
- Knowledge of community nursing an asset.
