About the Role

CarePartners is a leading provider of home health care services in Ontario, and we're seeking a skilled Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Visiting Nurse. In this role, you'll have the opportunity to make a meaningful difference in the lives of our patients, providing exceptional nursing services in a home setting.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide one-on-one nursing services to patients in their homes, including in-person and virtual visits
  • Communicate and collaborate with patients, family members, and other healthcare professionals to ensure seamless care
  • Perform other clinical duties as required

Requirements
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills and ability to communicate effectively with patients and healthcare professionals
  • A valid driver's license and vehicle, as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
