About the Role

VON Canada is seeking a Licensed Practical Nurse (LPN) to join our team in a Home Care and Visiting Care Agency setting. As an LPN, you will provide high-quality patient care and support to individuals in their homes, promoting independence and dignity.

Key Responsibilities
  • Assess and Plan Care: Conduct thorough assessments of patients' physical and emotional needs, develop and implement personalized care plans, and collaborate with healthcare teams to ensure comprehensive care.
  • Provide Skilled Care: Administer medications, perform wound care, and provide other skilled interventions as needed to promote patients' health and well-being.
  • Support Patients and Families: Educate patients and families on disease management, provide emotional support, and facilitate access to community resources.
  • Collaborate with Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and communication.
  • Document Patient Information: Accurately and thoroughly document patient information, including assessments, care plans, and progress notes.
Requirements
  • Education: Diploma or certificate in Practical Nursing from a recognized institution.
  • Experience: Minimum 1 year of experience as a Licensed Practical Nurse in a home care or visiting care agency setting.
  • Certifications and Licenses: Current LPN licensure in the province of practice, CPR certification, and other relevant certifications.
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong problem-solving and critical thinking skills.
