Healthcare Social Worker

Nova Scotia Health is seeking a skilled Health Care Social Worker to join our multidisciplinary team. As a key member of our team, you will provide a diverse range of interventions to clients and their families, enabling them to achieve their optimal level of functioning and health. Your expertise will be essential in psychosocial assessments, adjustment therapy, supportive therapy, crisis counseling, resource counseling, discharge planning, and case management.
You will be actively involved in advocacy, education, program planning, consultation, and collaboration on behalf of individuals and groups within the institution and community. Your role will be multifaceted and dynamic, requiring you to work autonomously as well as collaboratively.
About You
- You have a Master's Degree in Social Work from a recognized university.
- You are registered or eligible for registration with the NSCSW.
- You have 2 years of clinical experience as a clinical Social Worker.
- You have knowledge of the fundamentals of human behavior, including systems theory, family dynamics, group dynamics, and theories of behavior change.
- You are aware of local community resources and agencies.
- You have a valid Nova Scotia Driver's License and access to a reliable automobile.
- You have experience working with patients with complex chronic health conditions.
- You have experience co-facilitating education sessions with other multi-disciplinary team members.
- You have competencies in other languages, with French being an asset.
