**Job Summary:**

VON Canada is seeking a highly skilled Registered Practical Nurse to join our team in providing exceptional home health care services. As a Home Health Nurse, you will be responsible for delivering client-centered care to individuals and families in their homes and communities.

Key Responsibilities:

  • Assess clients' needs and develop individualized care plans in collaboration with healthcare teams.
  • Provide comprehensive nursing care, including physical assessments, symptom management, and education on self-care and independence.
  • Collaborate with clients, families, and caregivers to promote healthy behaviors and lifestyle changes.
  • Document all relevant information accurately and efficiently, ensuring compliance with VON standards.
  • Participate in continuous quality improvement initiatives and contribute to the development of health policies and nursing practice standards.
  • Engage with multidisciplinary teams and external organizations to promote best practices in nursing and community health.

Education, Designations, and Experience:

  • A valid license as a Registered or Licensed Practical Nurse in the province of practice.
  • Clinical practice experience is an asset, particularly in community health settings.

Required Skills and Qualifications:

  • Strong assessment and problem-solving skills, with the ability to adapt to changing situations.
  • Excellent communication and interpersonal skills, with a focus on building trust and rapport with clients and families.
  • Able to prioritize tasks and manage time effectively, ensuring timely completion of responsibilities.
  • Familiarity with adult learning principles and teaching methods, with the ability to provide health education and support to clients and families.
