Vernon Native Housing Society is committed to providing exceptional care to our community. We are seeking a skilled Registered Nurse to join our team as a Nurse Specialist - Indigenous Focused Complex Care.
About the Position
This is an exciting opportunity to work in a dynamic and innovative environment, creating a harm reduction program, focused on cultural safety and a trauma-informed approach to complex indigenous clients. As a Nurse Specialist, you will practice in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM).
The successful candidate will function as a member of the larger Complex Care Housing team and as a member of a designated multi-disciplinary team. Your role will involve assessment, treatment, case management, community support, and urgent response to clients who may also have serious functional impairment in the domains of physical health and personal, family, financial, occupational, and social life.
Key Responsibilities
- Provide intake, orientation, crisis stabilization, service bridging, and mental health and substance use assessments
- Identify primary issues and stage of change; collaboratively plan treatment goals and action plans with the client
- Use evidence-based individual/group counseling, crisis stabilization interventions, psycho-educational interventions, and other treatment modalities to promote client self-determination, strengthen readiness for change, and improve psychosocial functioning
- Facilitate clients' use of appropriate community resources by keeping knowledgeable about other services
- Offer consultation, support, and education to families, informal supports, community agencies, and other healthcare providers involved with CCH clients
- Advocate for clients by providing assistance in accessing community assets/services that contribute positively to the social determinants of health
- Respond to risk indicators that identify a need for clinical or legal intervention to ensure safety and well-being of an individual or others
- Provide assertive or planned outreach services, including emergency medication administration as required by the client population or individual client
- Recommend and facilitate hospital admissions as necessary/required
Requirements
- Graduation from an approved school of nursing
- Two (2) years' recent related experience in a mental health and substance use environment or an equivalent combination of education, training, and experience
