Job Description
As a Clinical Nurse Coordinator for the Liver Disease Program at the University Health Network, you will play a pivotal role in enhancing the quality of life for patients affected by liver disease. Your primary responsibilities will include:
1. Providing comprehensive education to patients and their families on lifestyle modifications, dietary guidelines, medication adherence, and potential complications.
2. Facilitating access to resources and support services for patients.
3. Maintaining accurate and timely documentation of patient interactions and intervention plans.
4. Performing Fibroscan tests.
5. Collaborating with staff hepatologists to optimize patient care pathways and outcomes.
6. Participating in quality improvement initiatives and data analysis.
7. Direct care responsibilities: performing venipuncture, IV insertion, albumin transfusions, paracentesis and thoracentesis procedures, wound care, recovering patients post-procedures, and liaising with clinicians regarding patient care.
Required Skills and Qualifications
To be successful in this role, you will require:
- Completion of a university, college, or other approved nursing program.
- Current Basic Cardiac Life Support certification.
- Current certificate of competency from the College of Nurses of Ontario.
- Physical Health Assessment Course preferred.
- Minimum of 2 years of clinical experience in hepatology, gastroenterology, or a related field.
- Certification in hepatology nursing or related specialty (preferred).
- Experience with phlebotomy, IV insertion, insertion of and access to vascular access lines, and administration of blood products.
- Demonstrated ability to exercise initiative and good judgment with the ability to multi-task.
- Ability to work as an integral member of a team.
- Excellent independent decision-making and problem-solving skills.
- Excellent organizational and time management skills.
- Excellent verbal, interpersonal, and written communication skills.
- Patient teaching skills.
- Ability to work effectively as a member of a multidisciplinary team.
