Job Summary:

CarePartners is seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team. As a Physiotherapist - Community Care Specialist, you will play a critical role in providing high-quality patient care in the community and in patients' homes.

Key Responsibilities:

Conduct professional assessments of patients in the community and in their homes
Develop and maintain effective Care Plans with respect to Community Health Services
Work with patients to establish time-specific, measurable goals and outcomes in collaboration with the care team
Provide active guidance for workplace Health and Safety
Perform other duties as required

Requirements:

Master's Degree (or equivalent) degree in Physiotherapy from a recognized university
Registration with the College of Physiotherapists of Ontario
Proven computer skills, including email, word processing, work scheduling systems
Strong assessment skills and the ability to participate in effective treatment programs
Excellent English verbal and written communication skills
French language skills are an asset
