Care Team Assistant

About the Role
The Care Team Assistant provides direct, hands-on care based on the needs of the patient and program/unit, reporting to the Health Services Manager and working under the supervision or direction of regulated health professionals.
About the Opportunity
This is a permanent, full-time position in the OPNZ Transitional Care Unit, requiring a Continuing Care Assistant Program and Provincial Certification with the Department of Health and Wellness.
Responsibilities
- Support patients' activities of daily living and instrumental activities of daily living
- Reinforce patient and family teaching according to established guidelines and the plan of care
Requirements
- Continuing Care Assistant Program
- Provincial Certification with Department of Health and Wellness
- Current Basic Life Support Certification
- Effective listening and communication skills
- Competencies in other languages an asset, French preferred
