VON Canada is seeking a highly skilled Registered Practical Nurse to join our team in providing exceptional care to individuals and families in their homes and community clinics. This is a full-time position with regular hours, including days, evenings, and weekends.

About the Role

The successful candidate will provide client-centered, evidence-based nursing care to individuals and families in their homes and community clinics. They will be responsible for assessing clients' needs, developing care plans, and implementing interventions to improve health outcomes. The Registered Practical Nurse will work collaboratively with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Provides nursing care to predictable clients with less complex care needs via an established plan of care.
  • Manages and facilitates care via an established nursing care plan and evaluates client responses to interventions.
  • Provides cost-effective, organized, outcome-directed care on an ongoing basis.
  • Conducts thorough assessments of clients and their environments using established procedures.
  • Obtains relevant information from clients, families, and healthcare teams to inform care decisions.
Requirements

To be considered for this role, candidates must have:

  • A current License as a Registered or Licensed Practical Nurse in the province of practice.
  • Experience in community health and clinical practice is an asset.
  • Strong written and verbal communication skills, with particular proficiency in interviewing stakeholders.
  • Ability to problem solve and adapt to changing care situations.
