**Job Description**
The Clinical Pharmacist plays a vital role in providing clinical pharmacy services, including individualized pharmacotherapy, patient medication counseling, and consultation with physicians and other healthcare providers to develop therapeutic plans.
Responsibilities include:
- Consulting with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plans
- Individualizing patient medication therapy by referencing standard literature, reviewing patient charts and laboratory test results, and assessing clinical responses
- Providing patient medication counseling regarding drug therapy, summarizing information, and meeting with patients and their families to provide and explain information and plans
- Assisting the healthcare team in developing drug therapy guidelines for individual disease conditions by reviewing current literature, compiling and analyzing information, and providing suggestions and recommendations
**Required Skills and Qualifications**
To be successful in this role, you will need:
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia
- Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience
- Broad knowledge of organization policies, procedures, and standards of care
- Knowledge of evidence-based clinical pharmacy practice, medication use evaluation, research processes, and methodology
- Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision
- Effective communication skills both verbally and in writing
