  • Conduct comprehensive client assessments, develop personalized care plans, and implement evidence-based practices to ensure optimal outcomes.
  • Collaborate with the care team to provide seamless support and transitions for our clients.
  • Support clients through care transitions by connecting them with community resources and services.

Requirements:

  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification an asset.
  • Enthusiasm for community care and passion for evidence-informed practice.
  • A current CPR certificate and updated immunization records.
  • A willingness to attain required Saint Elizabeth certifications.
  • Availability to work days, evenings, nights, and every other weekend.
