Job Description

As a Home Health Nurse with Champlain Rehab Solutions, you will be responsible for providing and coordinating home health nursing services to diverse individuals, families, and groups according to the College of Nurses of Ontario Practice Standards, VHA policies and procedures, and community health nursing standards.

This role requires driving to support clients with complex care and specialized nursing skills at home. You will assess, monitor, and make care planning/care coordination decisions, evaluate care plan interventions through on-going reassessment and evaluation results, and ensure quality of care by identifying and acting on factors affecting practice autonomy and delivery of quality care.

You will also demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families, support client safety care through continuous quality improvement, inform nursing supervisor of client activities and relevant concerns or changes, and maintain documentation standards according to the College of Nurses of Ontario.

Required Skills and Qualifications
  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Membership of your Professional Association preferred (RNAO/WeRPN)
  • RN diploma/degree or RPN diploma from a recognized university or college
  • Hold a valid First Aid or CPR Certification
  • A moderate level of wound care or venipuncture knowledge and experience is an asset
  • Acute Care and Medical-Surgical nursing experience is an asset
  • Knowledge of community nursing is an asset
